Houston, TX - RESPONSIBILITY: The Nurse Practitioner (NP) works autonomously but in collaboration with the primary care physician(s) in the delivery of primary medical care in the home. The NP provides medical services by managing acute and chronic conditions and developing treatment plans for patients at home and other resident settings which may include: assisted living, independent living, and senior housing.
